WHAT THE RESEARCH SHOWS

Acupuncture is the most rigorously studied modality in this library. For chronic pain, a landmark individual- patient-data meta-analysis (39 trials, 20,827 patients) found acupuncture outperformed both sham and no- acupuncture controls, with effects largely persisting at one year. A 2025 US trial designed to inform Medicare coverage found benefits in older adults. For the nervous system specifically, a 2023 meta-analysis of randomized trials found real (verum) acupuncture increased high-frequency HRV, a parasympathetic marker, while sham did not. A 2026 meta- analysis of 20 RCTs found manual acupuncture reduced anxiety compared with sham and usual care. Randomized trials also show improved sleep quality, including a sham-controlled trial in people with depression and insomnia. The evidence is honestly mixed in places. The Cochrane review of chronic low back pain found the difference versus sham was small and below the threshold for clinical importance, even though acupuncture beat no treatment.

A – StrongChronic pain: individual patient data meta-analysis of 20,827 patientsIndividual patient data meta-analysis · Tension / pain

IN PLAIN LANGUAGE

Pooling raw data from nearly 21,000 patients, acupuncture reduced chronic pain more than both sham needling and usual care, and most of the benefit was still present a year later. The authors conclude that referral for acupuncture is a reasonable option for chronic pain.

Study type
Individual patient data meta-analysis
Who was studied
39 high-quality RCTs, 20,827 patients (musculoskeletal, osteoarthritis, headache, shoulder pain)
Outcome area
Tension / pain
Key finding
Acupuncture was superior to both sham and no-acupuncture control for each pain condition (all P < .001). Treatment effects decreased only about 15% at one year.
Evidence grade
A – Strong
Limitations
Effect versus sham was modest (~0.2 SD); effect size depended on the type of control.

C – EmergingAcupuncture increases parasympathetic tone (HRV meta-analysis)Systematic review & meta-analysis of RCTs · Parasympathetic

IN PLAIN LANGUAGE

When researchers compared real acupuncture with placebo needling, only real acupuncture shifted heart rate variability toward the parasympathetic (rest-and-digest) side of the nervous system. More research is needed to confirm this.

Study type
Systematic review & meta-analysis of RCTs
Who was studied
9 RCTs comparing real vs placebo acupuncture
Outcome area
Parasympathetic
Key finding
Real acupuncture produced significant changes in HF and LF/HF heart rate variability, indicating increased parasympathetic tone. Sham groups showed no significant change.
Evidence grade
C – Emerging
Limitations
Only 9 RCTs; primary study quality and heterogeneity limit certainty (authors urge caution).

B – ModerateAcupuncture for anxiety: meta-analysis of 20 RCTsSystematic review & meta-analysis (PROSPERO-registered) · Tension / anxiety

IN PLAIN LANGUAGE

Across 20 randomized trials, acupuncture reduced anxiety more than placebo needling or usual care, with only minor, temporary side effects. The authors call it a promising, safe option and note that more long-term research is needed.

Study type
Systematic review & meta-analysis (PROSPERO-registered)
Who was studied
20 RCTs, 1,462 participants
Outcome area
Tension / anxiety
Key finding
Manual acupuncture reduced anxiety versus sham (SMD −1.06) and versus usual care/wait-list (SMD −1.35) after treatment. The effect versus sham persisted at follow-up. No severe adverse events.
Evidence grade
B – Moderate
Limitations
Very high heterogeneity versus sham (I² = 94%); follow-up benefit versus usual care not significant.

Who was studied
270 adults with insomnia and depression
Outcome area
Parasympathetic / sleep
Key finding
Sleep quality (PSQI) improved by 3.6 points more than sham and 5.1 points more than control at week 8, sustained to week 32. Anxiety and depression scores also improved.
Evidence grade
B – Moderate
Limitations
Conducted in 3 hospitals in one city (Shanghai); generalizability to other settings unclear.

C – EmergingAcupuncture for insomnia (meta-analysis of 24 RCTs)Systematic review & meta-analysis · Parasympathetic / sleep

IN PLAIN LANGUAGE

Pooled trials suggest acupuncture may improve sleep quality compared with sleep medication, with benefits typically showing up after three or more weeks of treatment.

Study type
Systematic review & meta-analysis
Who was studied
24 RCTs (15 in pooled analysis vs medication)
Outcome area
Parasympathetic / sleep
Key finding
Compared with sleep medication, acupuncture significantly improved PSQI scores, with benefits appearing after about 3 weeks of treatment.
Evidence grade
C – Emerging
Limitations
High heterogeneity (I² ~85–91%); many trials from a single region.

B – ModerateCochrane review: chronic low back pain (balancing evidence)Cochrane systematic review · Tension / pain

IN PLAIN LANGUAGE

The Cochrane review found acupuncture helped low back pain more than no treatment, but the difference compared with placebo needling was small. The authors suggest that cost, availability and patient preference can reasonably guide the choice.

Study type
Cochrane systematic review
Who was studied
33 studies, 8,270 participants
Outcome area
Tension / pain
Key finding
Acupuncture was more effective than no treatment for pain and function in the immediate term. Versus sham, the pain difference (about 9 points on a 100-point scale) did not reach the clinically important threshold.
Evidence grade
B – Moderate
Limitations
Moderate to very low certainty; most studies at high risk of performance bias.
